At the Department of Plant Pathology, we recognize that deciding to report an incident and choosing how to move forward can be difficult decisions. It is your choice to report an incident, and know that the JEDI committee will support you during this process. Note that you can always reach out to the Academic Advising Manager (Allee Hochmuth) for a conversation about your situation.
Which Reporting Option Do You Want to Access?
When to use this form?
When you want to anonymously report incidents or when you want support from trained professionals in Bias Response and Advocacy.

When you want to follow the campus-wide HIB guidelines.
How to use this form?
Simply click on the link and follow the instructions.
What to expect?
You will have to select a way to address the instance of HIB (interpersonal, administrative, formal), each with its own described protocol.
When to use this form?
When you want to either initiate a formal departmental complaint or request a conversation with a trusted faculty or member of the JEDI committee.

Simply fill out the departmental reporting system.
What to expect?
You will be contacted by the chair of the JEDI committee or a trusted faculty member of your choice to have a conversation about next steps based on your reported incident.
